You get the card by getting your medical records from you therapist or doctor stating what conditions you have, such as PTSD, fibromyalgia and you call and make an appointment with a doctor that approves with you diagnoses and you pay a fee of whatever that doctor charges and the you will pay a $50 fee for the ADH( Arkansas department of health)and they will approve you and you will get your card in the mail within 3 days.. hope this helps
